Before entering into a business relationship with a new client we ask that our 'Customer Maintenance Request Form' be filled, signed and returned to us before we proceed with your first order. The signed and dated Customer Maintenance Request Form should be returned to us with your Sales Order Form, Master, Artwork, Authorization Letter Form and Down payment.
